Abstract: Substance abuse is a social problem, not in India alone, but the entire world. The use of substance has its own culture and history, which varies from country to country. Substance abuse among adolescents has become a global challenge and also an important public health concern and for the past two decades there has been a dramatic increase in the demand for interventions to address the substance abuse problem. This demand has led to the development of multiple primary, secondary and tertiary substance abuse prevention programmes. June 26, 1992 was declared by WHO as the International Day against Substance abuse and Illicit Trafficking.

Abstract: The Glasgow Coma Scale or GCS is a neurological scale that aims to give a reliable, objective way of recording the conscious state of a person for initial as well as subsequent assessment. A patient is assessed against the criteria of the scale, and the resulting points give a patient score between 3 (indicating deep unconsciousness) and either 14 (original scale) or 15 (the more widely used modified or revised scale).GCS was initially used to assess level of consciousness after head injury, and the scale is now used by first aid, EMS, nurses and doctors as being applicable to all acute medical and trauma patients. In hospitals it is also used in monitoring chronic patients in intensive care. To evaluate the existing knowledge regarding Glasgow coma scale of staff nurses who are working in critical care unit.To find the effectiveness of planned teaching programme regarding Glasgow coma scale among the staff nurses of critical care unit.To find association between pre-test and selected variables of planned teaching programme regarding Glasgow coma scale among the staff nurses of critical care unit. An evaluative research approach with pre-experimental design was used. The sampling technique used was non - probability convenient sampling.  Data was collected from 50 staff nurses from Dhiraj hospital at Vadodara. In the post test mean score among staff nurses knowledge regarding Glasgow coma scale is 24.3 ± 3.95 higher than pre-test mean score 11.7 ± 3.60 among staff nurses regarding Glasgow coma scale, the mean posttest knowledge scores (24.3) was higher than the mean pretest knowledge scores (11.7).  The computed‘t’ value is 17.02 is higher than the tabled value t99 at p>0.001 level of significance.  Hence H1 hypothesis is accepted and thus shows that PTP was highly effective in increasing the knowledge of Glasgow coma scale. The result revealed that planned teaching programme was highly effective in improving knowledge of Glasgow coma scale in staff nurses. The overall experience of conducting this study was satisfying one as there was good co-operation from staff nurses and hospital authorities. The respondents were satisfied and happy with the information they received. The study was a new learning experience for the investigator. The results revealed that there is a great need for the staff nurses to update their knowledge regarding Glasgow coma scale. The study reveals that PTP could be used as an effective teaching strategy.

Title: A Pre-Experimental Study to Evaluate the Effectiveness of Planned Teaching Programme Regarding Knowledge of Glasgow Coma Scale among Staff Nurses Working In Dhiraj Hospitals Vadodara

Abstract: Introduction: The prevalence of smoking is rising in all developing countries, a trend seen in India also. Tobacco is killing around 3 million per year now, and the figure is expected to rise to almost 10 million per year by 2030. The average loss of life among smokers is around 8 years and respiratory morbidity is the most serious and common among all the health problems due to smoking. Materials and Methods: A case-control study was done during November and December of 2014, at a tertiary care teaching hospital in South Kerala. Cases were those adult patients who were admitted for respiratory complaints in General Medicine and Pulmonary medicine wards while controls were sex matched adult patients who were admitted for gastrointestinal complaints in the General Medicine or Gastroenterology wards. Results: There were 42 cases and 42 sex-matched controls in the study. Among the cases, 15(35.7%) were current smokers, 23(54.8%) were ex-smokers and 4(9.5%) were non-smokers, while among the controls a vast majority (28, 66.7%) were non-smokers. Among the cases, majority (37, 88.1%) were admitted for Chronic Obstructive Pulmonary Disease (COPD), while the others had hemoptysis due to some cause and lower respiratory tract infection as their treating diagnoses. Smokers were 19 times more likely to get admitted in a hospital due to respiratory complaints when compared to non-smokers. (OR-19.0, 95% CI- 5.6 to 63.9). Discussion and conclusions: This study points to towards the need to control the availability and use of beedis and cigarettes in the country. The governmental effort in this regard has to be stepped-up and programmatic flaws should be addressed.

Title: A study to find out the proportion of smokers among patients hospitalized with respiratory complaints, in a tertiary care hospital in south India

Abstract: Acute appendicitis is one of the commonest surgical emergencies. The risks of two primary outcomes must be balanced in the management of presumed appendicitis: perforation and misdiagnosis. Data was collected from 96 patients with complains of right iliac fossa pain over a period of 2 years in our hospital. All patients were categorised using the RIPASA SCORE. The preoperative and histological findings were compared with the preoperative diagnosis. Histopathologically 91 patients were in appendicitis groups and 5 patients were in no appendicitis group. On Evaluation of scoring system in the present study, RIPASA system was found sensitive (97.80%), specificity of RIPASA score (77%). Positive predictive value of score came out to be 98.89% in RIPASA. Negative predictive value of RIPASA system was 66.67%. Accuracy is 89.04% in RIPASA system. Predicitive negative appendectomy rate by application of score is  0.7% by RIPASA system. ROC analysis was done to depict the cut off with maximum sensitivity and specificity. RIPASA cut off was found to be 8.5 as compared to the original cut off 7.5. It was observed in the present study that there has been an increase in mean scores in the scoring system, with an increase in histopathological severity. Presence of gangrene was found statistically significant at RIPASA score ≥12. It can be concluded that there is high possibility to find gangrenous appendix when the RIPASA score are greater than 12.  On considering the above fact it is observed that RIPASA score is more accurate and more sensitive to diagnose acute appendicitis than other scoring system.

Abstract: We report a case of bacteremia due to Shewanella putrefaciens in 58yr old female patient who presented with high grade fever to hospital. She was on maintainace hemodialysis (HD) suffering from chronic renal failure. Blood culture from central and peripheral line was sent from which Shewanella putrefaciens was isolated which was resistance to most of commonly used antibiotic. Patient was started on imipenem as per result of antibiotic susceptibility pattern. She was recovered after five days of therapy and repeat blood culture was negative thereafter. Shewanella putrefaciens is resistance to routinely used antibiotic which make its identification and susceptibility essential.
