Abstract: Background: Since 2002, healthcare professionals have worked in primary care in the United Kingdom (UK), a transition that is also reflected in Australia, Canada and the United States. To understand how healthcare workers impact (or don't) on primary care workers, we conducted a case study. 

The documents include from the United Kingdom, Australia, Canada and the Americas, countries where the  role of healthcare professionals in primary care is well established.

Our findings highlight that health care workers are more likely to contribute effectively to the primary care workforce when they are supported to expand their current roles through formal education and clinical surveillance. We have also found that unless healthcare professionals are fully integrated into primary care services, they do not have the socialization necessary to build a relationship of trust with patients or physicians. doctor. Indeed, for patients to accept primary care health workers, their role and its implications for their care must be described by a reliable source. 

Conclusion: Our field review highlights the complexities surrounding the inclusion of health care workers in primary care roles.

In addition to providing insight into understanding the professional identity of healthcare professionals, we also discuss the range of expectations this professional group will face when transitioning to primary care.

This review is the first to provide insight into the impact health workers can have on the international primary health care workforce and how they can be deployed effectively. optimal way.

Abstract: In response to the new national medical education paradigm shifting; Mansoura Faculty of Medicine starts applying competency based medical education since 2018; one of the learning strategies it adopts is Case Based Learning (CBL). In this study we evaluated students` and faculty staff`s perception towards the effectiveness of this newly introduced method using a cross-sectional descriptive study on the starting cohort semester 2 students (no=1800) during the academic year 2021-2022 and the total number of academic staff member involved in conducting the sessions were 119. We used two validated self-administered online questionnaires formed of both open- and closed- ended questions. The data were analyzed; about near half of the students (47%) agreed that these sessions motivated them to learn, increase their long-lasting memory and helped them to correlate between clinical and basic science and also gave them opportunity to ask staff other questions related to their specialty, as well helped them to be in close contact with teaching staff, improved their communication skills and build their self-confidence. On the other hand, around 65% of the staff members enjoyed CBL sessions, agreed that the cases facilitated active discussion and interactions between staff and students and also improved the students’ ability to face others and the sessions were well organized. the overall perception from both students and staff was satisfactory and encourage us to seek for more improvements taking into consideration all comments they mentioned.

Abstract: The United States Department of Health and Human Services officially declared the ongoing opioid crisis a public health emergency in 2017. Due to the increasing over prescription of opioids for pain relief, the production of illicit alternatives, and the broken American healthcare system that could not prevent the spread of the opioid epidemic, the U.S. saw a constant, rapid surge of deaths from opioid overdoses. These opioids include a variety of commonly prescribed medications, but recent data and cases indicate that synthetic opioids are becoming the main contributor to these fatal overdoses. Synthetic opioids such as fentanyl are approved for medical purposes due to their designed potent properties for pain relief but are often misused due to false marketing by pharmaceutical companies that it produces a “safe and legal high.” Its potent properties are what get victims easily addicted and hence fatally overdose. This paper analyzes the role of synthetic opioids, specifically fentanyl and fentanyl analogs, and how it has shaped the opioid crisis and American public health.

Abstract: Background: Oral medication for patients with type 2 diabetes mellitus plays an important role in diabetes care and is associated with high levels of self-care and self-management behavior. Barriers to adherence can include complex treatment regimens, often associated with multiple long-term therapies, side effects from medications, and inadequate, incomprehensible, or confusing information or instructions from the healthcare provider. Multidisciplinary approaches can support successful adherence and enable more effective management of diabetes care. One The aim was to analyze the effectiveness of pharmaceutical interventions to improve adherence to therapy in type 2 diabetes mellitus. Study quality was assessed using the Cochrane Risk of Bias Tool. Two studies investigating primarily educational interventions showed a significant improvement in therapy adherence. In addition, the quality of the included studies was poor. Conclusion: Although pharmaceutical interventions could potentially improve medication adherence in type 2 diabetes mellitus, high-quality studies are needed to assess their effectiveness

Abstract: Astaxanthin is a type of carotenoid, which is a natural pigment found in a variety of foods. In particular, this beneficial pigment lends its vibrant red-orange colour to foods like krill, algae, salmon and lobster. Astaxanthin is an antioxidant. This effect might protect cells from damage. Astaxanthin also improves the way the immune system functions. Abstract: Vitamin D supplements are frequently introduced in infants according to medical guidelines. However, an overdose of vitamin D can lead to life-threatening hypercalcemia. We report the case of a 3-month-old infant with severe hypercalcemia. After asking the parents for details, an error in the use of vitamin D was identified. Indeed, the parents followed the advice of their midwives. They replaced the prescribed  vitamin D drug with a  different strength and dosage supplement without  the necessary dose conversion. In fact, there are many medications and supplements that contain vitamin D, which provide a variety of concentrations and units of measurement. This case highlights the central role of therapeutic education. In general, it is necessary to harmonize the regulation and labeling of dietary supplements and drugs containing vitamin D.

Abstract: Phyllodes tumor is a fast-growing tumor that has a big potential to be malignant. Malignant transformation of phyllodes tumors that were previously found to be benign has been reported, but there have been no reports stated transformation in such a short time. In this study, we reported a 34-year-old woman presented with complaints of a painless lump in her right breast, which had been felt since five years ago. Ultrasound examination and initial biopsy showed the breast lump as a benign phyllodes tumor. The patient then underwent a simple mastectomy with reconstruction, a clear safety margin, and axillary lymph node biopsy. Unfortunately, three months after the surgery, an additional breast lump reappeared.  Computed tomography scan of the chest with contrast and further histopathology showed a malignant phyllodes tumor of lung and bone metastases. The patient was then planned to undergo palliative chemotherapy and radiotherapy. Reflecting on this case, appropriate diagnostic steps and staging is a compulsory step before any prompt surgical intervention to determine the appropriate treatment.

Abstract: Introduction: Understanding the public knowledge and attitudes about HIV/AIDS is one of the cornerstones of tackling the disease. The most common problem people living with HIV/AIDS (PLWHA) face is the general public's lack of knowledge or misconceptions about the disease; which in turn leads to or precipitates discrimination against them, which leads to or precipitates problems with disclosure, social isolation, access to antiretroviral therapy, and psychological support. As a result, the goal of this study was to assess the knowledge, attitude and beliefs of population towards HIV in Medina, Saudi Arabia.

Methodology: A cross-sectional community-based descriptive study. The study was conducted in Al-Madinah ALMunawarrah -in 2023. Convenience sampling was implemented. A sample (n= 2006) were enrolled in this study. Data was collected through self-administrated questionnaire. Data was initially entered and cleaned using Microsoft excel, then analyzed using Statistical Package for Social Sciences (SPSS) v.26, descriptive statistics were used and categorical data presented in form of frequencies (n) and percentages (%).

Results: 2006 residents were included in this study, out of which, 1065(53.1%) of them were females. Majority had a bachelor degree 977(48.7%). Regarding knowledge assessment, analysis showed that the majority (59.3%) have poor level of knowledge. Only few of the participants were knowledgeable about the mode of transmission, where (11.4%) agreed that HIV can be transmitted through infected person cough/sneeze, (16.3%) agreed that sexual relations with infected person transmit HIV, and (12.7%) believe that HIV can be transmitted through blood products. Regarding the attitude, the majority of the participants (96.1%) have a poor level of attitude towards HIV disease. (87%) of participants think that HIV patients should be isolated from the community.

Conclusions: Majority of the participants included were females, with almost half of them holding a bachelor degree. The study concluded that the majority of participants showed poor level of knowledge towards HIV, as well as a negative level of attitude towards HIV.

Abstract: Background: Drug abuse is a social issue, it is not only in India, but the entire world has become a victim of it. The utilization of such wrong things has its very own way of life and history, which differs from nation to nation. The issue of drug misuse is developing at a hazardous rate and in only minimal over a decade it has spread its noxious appendages to pretty much all aspects of globe surmounting practically all hindrances of race, position, belief, religion, sex, education, and socio economic status.

 Aims: The aim of the study was to assess the knowledge regarding hazards of drug abuse among male students selected nursing colleges at Tumakuru.

Methodology: The knowledge of students was measured using structured questionnaire from a convenient sample of 60 male student studying at selected nursing colleges Tumakuru. In a descriptive survey design. Data were analyzed using descriptive and inferential statistics.

Findings The finding of the study indicate that the major findings of the study were out of 60 samples The findings on assessment of knowledge regarding hazards of drug abuse showed that majority 31 (51.66%) of the male students had average knowledge regarding hazards of drug abuse, 28 (46.66%) were having poor knowledge and only 1 (1.66%) had good knowledge regarding hazards of drug abuse.

Statistical analysis using chi-square test was computed to find out the association between knowledge scores male nursing students with selected demographic variables. The computed values were not significant at 0.05 level of significance. Hence, there was no statistically significant association between knowledge and selected demographic variables.

Conclusion: The study concludes that the majority of the male students had average knowledge regarding hazards of drug abuse. This study is effective to identify the knowledge among male students and how to overcome the hazards.

Abstract: This is a hospital-based observational study to estimate the prevalence of abnormal semen parameters in the northeast part of India and the effect of aging, lifestyle, and habits associated with it.

Methodology: This study was conducted among the male partner of the infertile couple who attended our infertility clinic from Jan 2019 to July 2021. After written consent semen samples were collected and analyzed in our andrology lab based on the 2010 WHO recommendation. For each male partner two consecutive sample collected one month apart and the study result was mean of two sample analysis. Result of seminal analysis, age, lifestyle, habits, and occupational exposure associated with different semen parameters were collected and analyzed by using SPSS Software version 22, with unpaired T test and Logistic regression analysis.

Result: In this study   14.8% males had oligospermia, 14.2% had Semen volume <1.5ml, and 51.8% males had total motility less than 40%, (62.2%) had progressive motility less than 32% and 23.3.% had abnormal  morphology <4% and 25.6% had vitality <58% .Semen volume was reduced in the2017478 age group more than 40 years (P value 0.000 ExpB 2.6) and with smoking  (ExpB1.99). Sperm count was significantly less in the age group > 40years and with smokers (P VALUE-0.000,.0.012). Smoking affects the total motility of sperms (ExpB 1.873). Progressive motility was affected by smoking and alcohol (P value 0.000,0.000 ExpB 2.19,2.02). Vitality was affected by smoking, alcohol, and in underweight BMI (p-value 0.010,0.041, 0.001,). Morphology was affected by smoking, alcohol, (p-value –0.000, 0.000, )

Conclusion: Increased age affect semen volume, and sperm count. Alcohol intake affects progressive motility, vitality and morphology.Smoking affect all parameter of semen,  low sperm  vitality was associated with low BMI.

Abstract: The urea cycle is a series of metabolic reactions that convert ammonia into urea to remove it from the body. Urea cycle disorders are characterized by hyperammonemia, which can cause irreversible damage to the central nervous system. We report a series of three infants presenting with irritability, poor feeding, and rapid breathing. Their first gas analysis revealed respiratory alkalosis. Hyperammonemia was confirmed and three different urea cycle enzyme blocks were diagnosed. Immediate treatments for include removal of ammonia by reducing catabolism, dietary modification, use of nitrogen scavengers, and finally dialysis. Hyperammonemia is a medical emergency that should not delay treatment. This report aims to emphasize the importance of suspecting a urea cycle disorder in neonates with nonspecific signs of hyperammonemia and respiratory alkalosis, and to summarize the main management directions for hyperammonemia.

Abstract: This study focused on the healthy workplace and employees’ wellness dimensions in the promotion of wellbeing and productivity at work in the new normal of the non-teaching employees of Laguna State Polytechnic University San Pablo City Campus. The study used descriptive method with an aid of spearman rho that identified significant relationships between healthy workplace towards employees’ wellbeing and productivity at work in the new normal and employees’ wellness dimensions towards wellbeing and productivity at work in the new normal. As to result, the respondents highly observed a healthy workplace, highly practiced the employees’ wellness, highly experienced the employees’ wellbeing at work, and highly experienced the employees’ productivity at work in the new normal. Therefore, it rejects the null hypothesis of the study. Hence, to support the university practices, the study proposes an improvement on the healthy workplace and employees’ wellness to promote wellbeing and productivity at work in the new normal and highly recommends an extension project entitled “Project Wellness”.

Abstract: This literary review provides a summary of the potential applications on the topic of full dive immersion and its applications in the field of neurorehabilitation. The review aims to explore the existing literature and research surrounding full dive immersion, examining its potential benefits, limitations, and future directions. Through comparing and cross-referencing existing methods of neurohabilitation as well as potential procedures of full dive immersion, this review hopes to examine the effectiveness of full dive immersion as a therapeutic tool for individuals with neurological disorders compared to traditional rehabilitation approaches. The findings of this literary review will contribute to a comprehensive understanding of the role and potential of full dive immersion in neurorehabilitation, guiding future research endeavors and informing clinical practices in the field. By synthesizing relevant studies and scholarly articles, this review seeks to shed light on the current state of knowledge and identify gaps in research which will contribute to a deeper understanding of the therapeutic potential of full dive immersion and its implications for neurorehabilitation.

Abstract: After the end of the 1^st wave of Covid-19 pandemic, Egypt decided that the new academic year (2020-2021) will be conducted via blended learning approach, the didactic lectures will be held online, and the small number activities will be face-to-face in the campus. This study aimed to evaluate and reflect on the satisfaction of students with the used blended learning approach in order to detect weakness and points to improve. A cross-sectional study was conducted on undergraduate medical students at Mansoura Manchester Medical Program. The sample was chosen in respect to clinical, preclinical years, male and females. A questionnaire was designed formed of closed & open-ended questions. The results were subjected to SPSS program for Windows and the results were considered significant when p ≤ 0.05. The younger students were more satisfied with blended learning and the results differences were mostly statically significant. The only two exceptions in which older students recognized model more organized & reported less problem during the beginning of E-learning. Also, female students were more positively perceiving blended learning in all items of the questionnaire.  Most of students asked for better time management, better technological system support and increasing time for clinical /practical rounds. They suggested also more staff training for E-learning and stated that internet connection is a major challenge for E-learning. Younger students and females are more in favour for the blended approach. Working on the multiple challenges as infrastructure and staff training is a must.
