Abstract: One of the many initiatives to reduce the amount of time spent waiting for a diagnosis and to improve patient outcomes is the establishment of guidelines for the urgent referral of patients who have a suspicion that they may have cancer. These guidelines were developed by a number of different organizations. The pelvic examination has been acknowledged as an essential component of the well-woman visit ever since ancient times. This session is seen by many women and gynecologic care providers as an opportunity to discuss sexual and reproductive health problems with their patients. Consequently, many women take advantage of this opportunity. A pelvic examination is typically performed on asymptomatic women as a screening tool for gynecologic cancer, infection, and asymptomatic pelvic inflammatory disease; despite evidence to the contrary, some obstetrician–gynecologists and patients consider it to be important in detecting subclinical disease. a pelvic examination is typically performed on asymptomatic women as a screening tool for gynecologic cancer, infection, and asymptomatic An evaluation of the patient's external genitalia, an examination of the patient's internal genitalia with the use of a speculum, rectovaginal examination, and bimanual palpation are all potential components of the pelvic examination. There is not enough evidence to back the claim that pelvic examination (PE) is helpful in the process of identifying gynecological cancer.

Abstract: 744 patients from 3 different clinical settings were evaluated as follows: unfractionated heparin (UFH) during or after cardiac surgery (n=100), UFH after orthopedic surgery (n=100, = 205) and molecular weight heparin (LMWH ) after orthopedic surgery (n = 439). In the activation assay, the frequency of HIT-IgG formation varied from a minimum of 3.2% in orthopedic patients receiving LMWH to a maximum of 20% in cardiac patients receiving UFH; per antigen assay, the corresponding frequencies ranged from 7.5% to 50%.

However, in patients who developed HIT-IgG and received UFH, the probability of HIT was higher in orthopedic patients than in cardiac patients (according to the activation assay: 52.6% vs. 5%; odds ratio 21.1). [95% CI, 2.2-962.8]; P = .001; nach Antigen-Assay: 34,5 % vs. 2,0 %; Odds Ratio, 25,8 [95 % KI, 3,2-1141]; P < 001). It is concluded that there is an unexpected dissociation between the frequency of HIT-IgG formation and the risk of HIT, which depends on the patient population.

Abstract: Estimating the magnitude and cost of antibiotic overuse in outpatient settings, and identifying the conditions responsible for the bulk of overuse, must guide interventions and policy decisions. Data from the 1998 National Survey of Ambulatory Medical Care, a sample survey of ambulatory medical practices in the United States, was used to estimate primary care visits and antibiotic prescription rates for acute respiratory infections. The weight-average cost of antibiotics was calculated usingprescription marketing data from 1996 and adjusted for inflation. In 1998, approximately 76 million physician visits for acute respiratory infections resulted in 41 million antibiotic prescriptions. Antibiotic prescriptions to treat bacterial infections exceeded the expected number by 55% (22).6 million) of all antibiotics prescribed for acute respiratory infections at a cost of ∼726 million US$. Upper respiratory tract infections (unspecified), pharyngitis and bronchitis were the conditions associated with the highest overuse. This study documents that the magnitude and cost of antibiotic overuse in acute respiratory infections by primary care physicians is significant and establishes potential target antibiotic treatment rates forselected conditions.

Abstract: Coronavirus disease 19 (COVID-19) is spreading rapidly in China and around the world. Pharmacy services are an important public health pillar to prevent and contain the COVID-19 pandemic. Saudi pharmacists have acted quickly in public relations work in KSA, such as: B. Developing a professional services guide for pharmacists and pharmacies, creating  emergency medication forms, monitoring and resolving drug shortages, setting up remote pharmacy services to treat  human infections prevent human, provide event-based pharmaceutical care, educate the public about infection prevention and  infection management, diseases,  and participate in clinical trials and drug evaluation. This comment addresses the unique needs of pharmacy services in the COVID-19 pandemic and shares our experiences with the international pharmaceutical community in the response to meet these requirements.

Abstract: By contrast, airborne pathogens and respiratory infections, whether seasonal influenza or COVID-19, are addressed fairly weakly, if at all, in terms of regulations, standards, and building design and operation, pertaining to the air we breathe. This could in part be based on the lack of perceived risk or on the assumption that there are more important ways to control infectious disease, despite ample evidence that healthy indoor environments with a substantially reduced pathogen count are essential for public health. Yet, before COVID-19, to the best of our knowledge, almost no engineering-based measures to limit community respiratory infection transmission had been employed in public buildings (excluding health care facilities) or transport infrastructure any- where in the world, despite the frequency of such infections and the large health bur- den and economic losses they cause (3). First, infection- focused ventilation rates must be risk-based rather than absolute, considering pathogen emission rates and the infectious dose [for which there exist data for a number of dis- eases, including influenza (6), severe acute respiratory syndrome coronavirus (SARS- CoV), Middle East respiratory syndrome, tuberculosis, SARS-CoV-2, and measles]. The only types of public buildings where airborne infection control exist are health care facilities, where requirements for ventilation rates are typically much higher than for other public buildings (9). Global WHO IAQ guidelines must be ex- tended to include airborne pathogens and to recognize the need to control the hazard of airborne transmission of respiratory infections.

Abstract: Brucellosis is a zoonotic infection that contaminates humans from animals with the infection by digestion of food products or by contact with tissue or fluids. It is the most common bacterial zoonosis globally. Brucellosis is a multisystem infection with a broad spectrum of clinical manifestations. Nonetheless, neurological system and cardiovascular association is very rare. Brucellosis is still a main public health problem and is prevalent in many countries, particularly in Mediterranean areas, parts of south and Central America, and East and Western Africa. It may affect any organ system. No cases were shared in the journals to describe chronic brucellosis with multisystem involvement, including epidimyitis, cutaneus, spondylodiscitis.

In this case report, it was aimed to share with the literature, a case of chronic brucellosis with multisystem involvement (epidimyitis, cutaneus, sponylodiscitis).

As overall, it is considered wise to think about brucellosis when dealing with patients with multi-system involvement of unknown etiology, particularly in regions where there is a high prevalence of brucellosis.  In similar cases, patients should be carefully monitored for any evidence of complications such as multi-system involvement. In addition to other systems such as central-nervous and cardiovascular systems.

The treating physician must keep suspicion of any complication that can occur. High and meticulously look for all possible complications should be done in advance, because once they occur, quick decision taking should be done in order to save patients’ life.

Abstract: Health Care Associated Infections (HCAI) are a major cause of morbidity and mortality in patients. Hand hygiene is pivotal to controlling these infections. The perception and practice of hand hygiene among clinical medical students of Chukwuemeka Odumegwu Ojukwu University, Anambra State, Nigeria were studied. Many knew that infections can be transmitted between patients through the hands of healthcare workers and students (95.6%). Only 63.0% had received formal training on hand washing. Some believe that wearing gloves reduces the need for hand hygiene (44.5%). Those that practice hand hygiene before and after touching patients were 17.0% and 43.7%, respectively. Reasons given for not practicing hand hygiene adequately were lack of water (88.1%), lack of sanitizer (76.3%), forgetfulness (79.3%) and insufficient time as they had many things to attend to (54.1%). Medical students should receive training on hand hygiene, and facilities for its practice should be adequately provided.

Abstract: Child health is fundamental to adult health and wellbeing. Physical activity during childhood is one of the important aspects which promote better health. Physical activity is defined as any bodily movement produced by skeletal muscles that require energy expenditure. Health problems associated with decreased physical activity such as overweight / obesity, poor oral health, diabetes and other chronic physical and mental health problems among adolescence are increasing worldwide. In the present study data was collected on knowledge regarding health problems due to decreased physical activity among secondary school children in a selected school at Tumkur. A quasi experimental one group pre-test post-test design was used for this study. The study was carried out in selected school at Tumkur, Karnataka. The sample comprised of 100 school children studying in 8^th, 9^th and 10^th standard in a selected school at Tumkur. The students were selected by stratified random sampling. The pre-test was followed by the implementation of video assisted teaching programme and post test was conducted after 7 days using the same structured questionnaire.  The results were described by using descriptive and inferential statistical analysis. Results of the study revealed that the overall post- test mean knowledge score19.26 (64.2%.) were significantly higher than the overall mean pre-test knowledge scores 11.33 (37.77%) and the obtained ’t’ value 27.749 is greater than the table value at 0.05 level of significance. The findings of this study support that video assisted teaching was effective in improving the knowledge of the secondary school children regarding health problems due to decreased physical activity.
