Abstract: A number of reports have suggested the benefits of the acute care surgery (ACS) design in surgical results. We aimed to mark the impact of the ACS model on surgical effectiveness and quality.Before the ACS model was executed, abdominal surgical emergencies were evaluated by an on-call non-trauma basic cosmetic surgeon (pre-ACS model). An in-house injury cosmetic surgeon dealt with all patients with injury or non-trauma abdominal surgical emergencies after the ACS design. Patients with acute appendicitis who went through appendectomies were consisted of. We conducted a pre- and post research study to compare the time patients remained in the emergency department and surgical qualities.The ACS model might improve abdominal surgical performance and quality. Abstract: Diabetes is a growing health problem globally and particularly in Saudi Arabia. Unfortunately DM considered to be one of the most common cause for serious complications (either acute or chronic) that affecting a lot of organs all over the body. Public awareness and education about diabetes and its complications is the cornerstone in prevent progression of the disease and developing of complications. SO, considering this issue we conducted this study to asses school teachers knowledge and their attitude toward DM complications in Tabuk city while no similar study had been done in our region. Methods: cross sectional study using structured self-administered questionnaire (containing 17 questions) distributed in random way among teachers at different schools in Tabuk city containing socio-demographics, attitude and knowledge toward DM complications. Anonymity of respondents, consent form, and other ethical issues were considered. The data gatherers were available to answer any questions. The target population size was 500, based on 50% prevalence and 95% confidence interval. The data collected from these questionnaires was entered in Microsoft Excel and the statistical analysis was done using SPSS version 21. Frequency tables and chi square test analysis was done. Results: A total of 500 respondents were interviewed, 250 (50%) were male . only around 20 of them are not in 20-40 years age group and most of them are married and had bachelor degree. Hopefully 86 % of them believe that DM has serious complications as same as almost 377 ( 75% ) of them follow malty ways together ( medications , regular F/U, diet and life style modifications,..) to minimizing DM complications. Surprisingly, only 11% of them have no DM in their relatives. More than 50% are taking DM information's mainly from health providers and social medias. Half of our respondents think that there are many social difficulty among diabetic patients in their life and more than 450 out of 500 participants (93%) they complain of shortage of educational project towards DM complications . the level of agreement from our respondents Regarding the retinopathy, cardiovascular, nephropathy and peripheral neuropathy was (89.6%, 75.2%, 75.4%, 85% ) respectively. 36 teachers (7.2%) don’t believe that DM has role in foot amputation, approximately 14% and 18 % of total sample size don’t know about diabetes effect on sexual activity and blood pressure . Conclusion: The participants have somehow adequate knowledge of DM complications. But they are still in need effective health promotion strategies for the educational institutions and health program and camps should be conducted to make aware of community about the DM complications and how to try to minimize it . Abstract: This review study was aimed to overview the diagnostic procedures and treatment approaches of acute otitis media (AOM), through reviewing the evidence based on the proper treatment for this otolaryngology condition. We carried out an extensive literature review, through following databases; (Medline, and Embase) searching for articles presenting reporting diagnosis and treatment of AOM, published up to January,2017. Search restriction was applied to English language articles with human subject included. We further searched references lists of those selected articles. Acute otitis media and otitis media with effusion are common youth disorders, a source of considerable morbidity, and a leading reason for antibiotic prescription in primary health care. Effective treatments are offered, some imperfections stay, and thus better treatments would be welcome. Recent discoveries within the field of otitis media research relating to its etiology and pathogenesis have actually caused additional investigation focused on developing novel treatments. Abstract: Locked-in syndrome is a rare neurological disorder in which there is complete paralysis of all voluntary muscles except for the ones that control the movements of the eyes. We reported the case of is a 42 year old gentleman, not known to have any medical illnesses Admitted to ER complain of dizziness and slurred speech for 2 hours, and then he had episodes of jerky movement (seizure like) for few minutes, without loss of consciousness. Abstract: Aim of this study is to estimate the Prevalence of poor quality of sleep among adolescents in Jeddah during summer vacation, to know to which extent do these factors affect the quality of sleep among Jeddah’s adolescent population during summer vacation and To compare multiple factors and its effect on the sleep quality. Methods: A cross-sectional study was conducted in 2016 in Jeddah, Saudi Arabia. Adolescents were evaluated by online Google form distributed through social networking, between August 2016 to September 2016. The questions were divided to demographic data, questions to assess sleep quality according to Pittsburgh Sleep Quality Index (PSQI), and questions to assess each specific factor (diet, obesity, video games, TV, social life, chronic medical illness, and physical activity). Quality of sleep was determined by using Sleep Quality Index (PSQI) which it divided into 2 categories: good (A total score of less than 5 ) and poor (A total score of 5 or greater). Demographic/lifestyle factors were collected and analyzed as factor affecting quality of sleep. Rusults: Atotal of 202 respond to the electronic questioner and the result showing that females represented 56.4% of the sample. About 79% of 62 responds sleep in a dark room, 67.5% of 80 responds have TV in their bedroom and 55% of 169 responds had more than 3 social gathering in the past week. Around 92% of the adolescent were found to have a PSQI global score of >5, which reflects Poor quality of sleep (Figure 1). Poor quality of sleep was significantly higher in adolescent using electronic devices before sleep (P = 0.001), not drinks milk before sleep (P = 0.001) and self-assessment (P = 0.005) .(Table 2). While there is no significant association between quality of sleep and other factors such as gender, Education, Having Dinner ,Vitamin B supplements, Type of summer activities ,Meting people,Have TV in bedroom, Sharing bedroom, Sleeping with light on, Sleeping in noise and Exercise last week. Conclusion: Poor quality of sleep is associated with up to 92% of adolescent in Saudi Arabia during summer vacation, which is high by comparison to other studies. Abstract: At the turn of the 21st century, specialized units offering care to HIV/AIDS patients became necessary. A regular assessment and appraisal to assess workers in these special units is necessary to gauge their satisfaction and ability to deliver high quality care to patients and address concerns, conflicts and expectations among them as they interact with a unique patient cohort suffering from a stigmatized and currently incurable illness. Aim: To assess employee view on their work environment and their outlook on the organizational precepts of leadership and planning, corporate culture, work place communication, teamwork, working conditions, career development, supervision, and training program among other workplace aspects. Methods: This was a questionnaire based cross sectional survey using the Likert scale on level of agreement or disagreement to a set of questions, and involved both the contracted and permanent employees working at the HIV/AIDS care unit of a large tertiary hospital in Kenya. Results: 47 out of a total employee workforce of 56 participated in the survey, reflecting a response rate of 84%. The 4 unit administrators and the 3 ‘volunteer’ peer educators were left out of the study. An overall satisfaction assessment of the entire workplace concerns showed that most of the employees were satisfied (57.4%, n=27) as compared to only 6.4% (n=3) who were dissatisfied and a neutral group at 36.2% (n=17). Conclusion: Staff satisfaction assessment is a necessary component in human resource engagement in any organization. Workers in certain units in an organization may have certain special concerns that need to be addressed regularly to enhance their productivity. Abstract: The main purpose of this article was to review the surgical repair procedures for medial collateral knee ligament (MCL), but first anatomy and diagnostic approaches must be discussed to give a fully understand to the operative techniques. Online search was performed using a computer-based search through the well-known medical databases: PubMed, and Medline, to identify all publications describing repair of the MCL of the knee, until January, 2017. Electronic search was limited by study of English language of publication and human subject articles. MCL and medial-sided knee injuries are still significant issues in the modern-day age of sports medicine. With advances in imaging techniques and refined grading of injuries, surgical treatment for medial-sided knee injuries, present review showed that repair of the MCL and posteromedial corner of the knee might be a dependable and efficient treatment for medial-sided knee injuries, resulting in enhanced valgus stability and patient-reported functional ratings with low rates of secondary failure. English language restrictions were applied with human subjects only. References from identified articles were searched for additional relevant studies. Abstract: A search was conducted through following databases, PubMed/MedLine, and Embase for relavant articles published in English language and involving only human subjects, up to December 2016. Searching through evidence was performed using MeSH terms as following: “pancreatic pseudocyst, pseudocyst drainage, cystogastrostomy, cystojejunostomy, transmural pseudocyst drainage, transpapillary pseudocyst drainage and percutaneous pseudocyst drainage”. Reference lists from eligible trials were searched for more evidence to be included and useful in our review. The aim of the current review paper was to evaluate the outcomes different surgical procedures for managing Pancreatic pseudocyst, however comparatives studies on different techniques were involved in this article. Pancreatic pseudocysts are a common issue of chronic and acute pancreatitis. They are diagnosed with imaging studies. Pancreatic pseudocysts are generally handled by open surgical internal drainage. With continued improvements in medical technology, making uses of percutaneous, laparoscopic and endoscopic drain were significantly reported. Endoscopic pseudocyst drain has a high success rate and a low complication rate. Percutaneous drainage is primarily utilized for the first aid of contaminated pancreatic pseudocysts. Abstract: This review aimed to focus in discussing the Bipolar disorder from different psychological aspects, but mainly emphasizing the proper treatment approaches of this disorder, in addition to diagnostic procedures that can be done to evaluate the Bipolar disorder. Comprehensive search of literature was performed using MEDLINE/PubMed and Google Scholar, and was restricted to studies published in the past up to the November, 2016. The search terms included “bipolar disorder,” “mania,” “bipolar depression,” “mood stabilizer,” “atypical antipsychotics,” and “antidepressants,” and “treatment,” or “therapy.” we included studies that focus on diagnosis, treatment, of bipolar disorder, articles were selected with restriction to English language and human subject’s inclusion. References found in each study were manually searched for more relevant articles to be included in this review. Accurate medical diagnosis and prompt, appropriate treatment are presently our best wish for decreasing this disease problem. It is for that reason important that clinicians thoroughly screen depressed patients for bipolar disorder and understand confusing comorbid disorders, such as anxiety disorders and substance abuse. The possibility of manic episodes in undiagnosed bipolar disorder treated with antidepressant monotherapy must also be kept in mind, given that these episodes may assist point the clinician toward a right diagnosis. Since bipolar depression is often refractory to treatment, aggressive therapy might be required. Abstract: Diabetes mellitus is a physiological condition caused by either production of inactive insulin or inadequate production of insulin. Insulin is responsible for regulating blood sugar levels. Objectives: This study aim to unveil how well informed the population are as far as diabetes mellitus is concerned. Methods: A total of 400 subjects were randomly selected and interviewed using two different structured questionnaires during the period from August 30, 2016 to September 17, 2016. The questionnaire were including risk factors, symptoms and complications of type 2 diabetes mellitus as well as methods of prevention The SPSS (version 20) was used for statistical analysis. Results: Out of 400 subjects 200 were males and 200 were female, their ages ranged from 18 to 24 years old and 16.5% (66/400) were diabetic patient. In this study, 52.5% (105/200) of the male respondents were not aware of the risk factors of diabetes mellitus. This was a larger number compared to the 40.5% (81/200) of female unaware. However, 92% of both male and female respondents taking health related courses had an idea about causes of diabetes mellitus. About 67% (268/400) of the respondents were not sure whether the condition is infectious or non-communicable. Only 29.25% (117/400) of participants identified high blood pressure as major risk factor. Lifestyle and genetics were the major aspects most of the respondents were aware of causes of diabetes mellitus. Just 9.5% (38/400) knew that some medications can cause the disease and close to 60% (240/400) were not aware of the methods of prevention. 39.5% (158/400) knew that diabetes mellitus can result peripheral neuropathy while 30% (120/400) knew that diabetes mellitus can cause retinopathy. Other complications got low response including chronic renal failure 12.5% (50/400), CHD 11% (44/400), sexual dysfunction 4.5% (18/400) and stroke 4.5% (18/400). Conclusion: There is lack of awareness of diabetes mellitus among Saudi population. This calls for larger studies to investigate the awareness of this morbid disease among the public. Abstract: This review study was aimed to address and evaluate the roles of Fibrinolysis in Enhance Fracture Healing, moreover to discuss the evidence based trails discussing this topic. Comprehensive search was conducted through; PubMed, the Cochrane Library, and Embase for studies published in English language up to December 2016, and human and animal subjects, search was performed using the following combination of terms: “Fibrin, Fibrinogen, Fibrinolysis, Fracture Healing, Plasminogen”. studies which were discussing the roles of Fibrinolysis in Enhance Fracture Healing were included in this review. Evidence reviews in this study showed that, fibrin was completely dispensable for long-bone crack fixing, as recovery fractures in fibrinogen-deficient mice were identical from those in control animals.
