Abstract: Main objective of this review was to emphasize the importance of the electrical cardioversion in Emergency atrial fibrillation management, we also intended to review the other methods of atrial fibrillation management in ED. A conducted search of literature using electronic databases, including Medline, EMBASE, and Cochrane for studies discussing the electrical cardioversion for management atrial fibrillation in Emergency department published earlier up to 2017. Not all patients with acute atrial fibrillation presenting to an emergency department require medical facility admission. It is possible to safely handle and release a part of patients with acute atrial fibrillation in an emergency division observation unit or professional decision system. Management needs to be individualized to each patient relying on numerous factors including patient signs and symptoms. Electric cardioversion of atrial fibrillation in an outpatient setting with a surveillance for at least 3 h is possible in a selected populace with a reduced threat. Patients with a much shorter AF-episode appear to have a much better end result in a worrying quality of life. In spite of a substantial recurrence rate electrical cardioversion has a high patient-acceptance. A high percentage of patients chooses cardioversion in an outpatient setting.

Abstract: Ocular morbidity is a major cause of global visual impairment and blindness. The magnitude of global burden of visual impairment and blindness associated with industrial and occupational activities is of great public health importance. To a large extent, factors such as the state of the work environment, type of occupation, level of income and education, and patterns of socialization have considerable impacts on ocular health. The objective of this research was to study the socio-economic variables associated with ocular morbidity due to industrial activities. 150 volunteers presented themselves for this study. Non-probability sampling method was used. The research design used was a cross-sectional study, involving an on–site field screening of industrial workers. Morbidity was both reported and measured using clinical ocular examination procedures of the external eye and the posterior segment. Information on socio-demographic characteristics was obtained using interview-based examination administered by the examiner to each worker. Data obtained was presented using tables. Data was also tabulated and statistically analyzed. The tabular representation of data obtained was chosen for easy under-standing, interpretation and comparison of findings. Statistical package, χ2-test was employed for the analysis of data obtained. In this study, the prevalence of ocular morbidity was found to be 96.6%, with a significantly higher prevalence in males than females. The ocular morbidity observed was found to be significantly associated with levels of education, type of industrial activity, work environment, level of income, length of service (i.e. duration of work-related activity. Measures should be undertaken to render eye health education and creation of eye health awareness in industrial settings and in the community in general.

Abstract: The purpose of this study was to explore and describe clients’ experiences of primary healthcare services in six selected primary healthcare facilities located in Mwanza districts, Tanzania.

A qualitative appraisal using focus group discussions was used to collect data.  Qualitative content analysis was employed to analyze data. Data collection and analysis was guided by elements of quality care regarding staff punctuality, queue system, staff attitudes, treatment availability, information provision and opportunity for clients to provide feedback; guided data collection and analysis.

A total of 59 informants were included. Majority (64.4%) were female. More than half (54.2%) were young adults (20-40 years) with 39% of them to be between 41-60 years old. Fewer (6.8%) were 60 years and above. Results from focus group discussions yielded 6 main categories. The category “punctuality” showed informants’ need for staff to utilize their time better. The “attitude” category described under its subcategory “role of language” showed informants to be dissatisfied with language barriers when health care providers used a national language Kiswahili instead of native Sukuma.  “respect to clients” and “empathetic” subcategories emerged to indicate satisfaction in relationship aspects. “Unavailability” of treatment “costs too high”, “favoritisms,” “HCW always in a hurry” and “no opportunity to provide feedback” emerged as major issues that needed improvement. The study demonstrated an overall negative experiences on the care provided. The healthcare facilities need to ensure health providers adhere to professional communication skills, provide compassionate care, treatment and honor clients’ opportunity to give feedback for improved primary healthcare services.

Abstract: The bonding of orthodontic appliances to teeth commonly results in pain and discomfort. To alleviate the pain, analgesics, such as non-steroidal anti-inflammatory drugs (NSAIDs), are frequently prescribed. However, orthodontists and the parents of patients are equally concerned that pain relief medications may be misused or toxic, so seek safer, effective pain-alleviating interventions; these include non-pharmacological treatments. To evaluate the efficacy of various alternative interventions in reducing pain associated with orthodontic treatments, a well-designed randomised clinical need to be undertaken.

Abstract: Stress is a state of mental and emotional strain or tension resulting from adverse or demanding circumstances. There are both good and bad or positive and negative stress. 86% of Indian currency was abrogated in effort to Demonetization. The survey that 51% work and 50% personal finances are the contributing factors for the increased level of stress in the individual. Aims and Objectives: The main aim to conduct this study was to find out the level of stress regarding the Demonetization of Rs. 500 and Rs. 1000 notes among the people.

Material and Method: In this research study a Quantitative survey approach with Descriptive research design is used. Non probability convenience sampling technique was used to select the samples from selected areas of Vadodara District and data collection was done. Data was analyzed by using descriptive and inferential statistics such as Standard deviation and Chi-square test.

Result: In this study, 7% were having Mild Stress, 76% were having Moderate Stress and only 17% were having Severe Stress. Here one demographic variable living area is significant with the level of Stress.

Conclusion: In this research study findings show that the people have Severe and Moderate Stress regarding Demonetization of Rs. 500 and Rs. 1000 Notes.

Abstract: A prospective patient-blindclinical trial was carried in 20 candidates (patellar denervation in the right knee using circumferential cauterization, besides classical patelloplasty; and no denervation/patelloplasty to left knee) of simultaneous bilateral total knee arthroplasty (TKA) for end-stage osteoarthritis to assess the efficacy of patellar denervation in reducing anterior knee pain (AKP) following TKA. Presence, intensity and functional impact of AKP was compared between denervated (right) and non-denervated (left) knee using physical examination, visual analogous scale (VAS) and Kujala score, respectively; in addition to quadriceps diameter and sustained isometric exercise. Evolution over time of AKP was compared by 2-time assessment of pain intensity (< 1 year versus >1 year) using VAS. Eighteen patients completed the study. Prevalence (95%CI) of self-reported AKP was 22.2%; physical examination revealed that 61.1% patients had pain on patellofemoral compression; 16.7% had tender medial facet, and tender femoral grove each. No differences between denerveted and non-denerveted knees regarding pain intensity (VAS score=5.00±2.83 versus 5.11±2.72; p=0.317) or knee function (Kujala score=65.94±7.73 versus 67.17±11.60; p=0.346) were observed, respectively. Other assessments showed no difference in quadriceps diameter (p=0.375) or sustained isometric exercise (p=0.494). A significant decrease in pain was found over time in both denervated and non denervated knees, with no difference in change in pain intensity.This trial showed no efficacy of patellar denervation in reducing AKP following bilateral-TKA. Further trials are warranted to verify this result.

Abstract: Floating knee injuries mostly comes together with other severe injuries. Our study aims to assess the management and treatment of this injury, associated injuries, method and results of treatment, and complications of floating knee are discussed. We conducted computerized search among electronic databases; EMBASE, COCHRANE and MEDLINE for literature available in the databases to September,2017 using the following search term including variants: “floating knee”, “surgery”, “Management”, “treatment”.  Floating knee, with ipsilateral femoral and also tibial fracture, is a severe injury, mostly related to high-energy injury and also to numerous linked injuries, which are commonly very serious, as well as both systemic and local. In planning therapy, it is important to think about the patient's general problem as well as the situation of the limbs. Sometimes, amputation good alternative option. It’s very important to consider injuries and complications before deciding the plan for surgery. That’s, these injuries require management by an experienced multidisciplinary team.

Abstract: In this review, we aimed to look at the new pathomechanism, dysfunction and management of macrophage activation syndrome. Our aim is to summarize the progress in understanding of MAS pathophysiology, show the symptoms that can help find specific diagnostic method. A systematic literature search was performed in MEDLINE, EMBASE and Cochrane. Our computerized search strategy with English language restriction was conducted on literature of the past 25 years. Macrophage activation syndrome is a life-threatening complication seen predominantly in children with systemic onset juvenile idiopathic arthritis. It accounts for a significant amount of the morbidity and mortality, it is essentially important to recognize and treat MAS earlier in order to lower the mortality. There are still no validated diagnostic criteria and early diagnosis is difficult. Further immunological and genetic studies are needed in larger cohorts of patients in order to better understand the pathophysiology of MAS.

Abstract: Heart is the sensitive organ to hypoxia and in response to low oxygen availability numerous adaptive responses are initiated at the molecular, cellular and organ level. Cardiomyocytes increase their cardioprotective capacity in response to hypoxic conditions to survive the stress conditions. There are different possibilities, like increased transcriptions or translation which will account for the increased cardioprotective capacity of cardiomyocytes under hypoxic conditions. However, none of these account fully with the observed increase in the cardioprotective activity of cardiomyocytes. Moreover, it is well known that during hypoxia the cap dependent protein translation is decreased, but still expression levels of few cardioprotective mRNAs are enhanced. Cap-independent translation, mediated by Internal Ribosome Entry Sites (IRES) is an alternative strategy used by cells to maintain the translation rate of some mRNAs under stress conditions, like hypoxia. Thus we hypothesize that cap-independent translation may play a critical role in enhancing the cardioprotective role of cardiomyocytes under hypoxic conditions.

Abstract: Apixaban is an Anti-coagulant which prevents or reduces the coagulation of blood manifested by prolonging the clotting time. In the present study, a rapid, sensitive and economical UPLC method was developed and fully validated for analysis of Apixaban and its impurities (MPD4, APIX1B, N-Formyl and Methyl ester) in pharmaceutical finished product. Challenging task of separation of highly polar and non-polar impurities simultaneously with good resolution by development of accurate and sensitive UPLC method. The final UPLC method was developed by using Fortis Speed Core C18 column (150 mm × 4.6 mm, 2.6 µm). The temperature of column compartment was maintained at 40 oC. A gradient elution at a flow rate of 0.5 mL/min was employed and detection was carried out at 235 nm. Injection volume of 3 µL was employed. Eluent A : Mixture of 10 mM potassium dihydrogen phosphate, pH adjusted to 5.0 and Methanol in the ratio (90:10 v/v respectively) and eluent B : mixture of 10 mM Potassium dihydrogen phosphate, pH adjusted to 5.0, Acetonitrile, Methanol in the ratio (20:20:60 v/v/v). The validation parameters were inaccordance with FDA and ICH specifications, showing accuracy, precision, specificity, robustness and linearity from 0.1 – 2.5 µg/mL for the Apixaban and its impurities. The limit of detection and limit of Quantitation were Acid impurity and MPD4 impurity (LOD: 0.002%, LOQ: 0.007%), APIX1B impurity (LOD: 0.003%, LOQ: 0.009%), Apixaban (LOD: 0.002%, LOQ: 0.005%), N-formyl impurity (LOD: 0.001%, LOQ: 0.003%), Methyl ester impurity (LOD: 0.001%, LOQ: 0.002%) respectively. The validated method is simple, rapid, linear, precise, robust and method is suitable for Quality control applications.

Provided that HPV is the cause for almost all cervical cancers. Since it does not always show symptoms, it is important to check on existence of HPV virus. As the main transmission of this virus is through sexual interaction, it’s possible to prevent through vaccination especially in teenage.  It is expected that HPV vaccines will eventually decrease cervical and also other genital system premalignant and malignant illness. Good-organized programs of regular gynecological screening and treatment of precancerous lesions have been very effective in preventing squamous cervical cancer.

A search was conducted of MEDLINE, EMBASE, and Cochrane Central for English-language published d studies up to, September, 2017. Search strategy were concerned with those articles discussing the diagnosis of depression by family physicians in primary care setting. The recognition, as well as treatment of depression, is a challenging for family physicians in primary care where there are many patients with numerous discussions and a multitude of causes for distress. An analysis difficulty could be seen in the distinction of a depression with dominating somatic symptoms from stress and anxiety, somatoform problems, as well as clinical conditions. We conducted a Comprehensive search through electronic databases; Medline, Embase, and Scopus for published articles concerning the obesity diagnosis in primary care up to September, 2017. Family doctors should record obesity as a diagnosis or formulate an excessive weight management strategy in the majority of patients. Abstract: Our main objective in the present review was to overview and discuss the impact of alcohol consumption on gastrointestinal tract, and digestive system in general. We conducted a narrative review over the literature using electronic databases as; MEDLINE, and EMBASE for studies involving data on effect of alcohol on GI tract, published in English language up to September, 2017. we then reviewed the references lists of included studies to find more relevant articles to be for additional evidence. Understanding the effects of alcohol on the GI system along with the underlying pathogenic mechanism(s) is critical for appropriate management of alcohol-related disorders. Such results of alcohol on GI could alter mobility and also transit. Esophageal dysmotility and also postponed stomach draining have actually been observed with high concentrations of alcohol in speculative researches and in chronic alcoholics. This impact of ethanol is not seen in all topics and also reverses with abstaining. Both problems might add to diarrhea as reduced transit decreases absorption whilst extended transit inclines to microbial overgrowth.
